Get My Forms Completed

Our Doctors will sign and complete various forms on your behalf. Please be aware that these often carry a charge. The receptionist can advise you if you wish to know the cost of your particular form.

Please hand your form into reception at Brixham or post it to our Brixham address. If you hand a form in to reception at Galmpton, the receptionist will send your form to Brixham to be actioned.

Please be ware that forms cannot be treated as priority over consulting ill patients but the doctor will endeavour to complete them within 10 working days of receipt at the Brixham surgery.

The most common forms will fill in are:

- Private Sick Notes
- Driving Licence Forms
- Insurance Reports
- HGV Licence Forms
