Obtain a Sick Note

Medical Certificates
For the first seven days that you are absent from work a self certification form should be completed. These forms are often available from your employer. If they are not available or you are self employed then you can obtain one from reception at either of our sites or a post office.

If you need a certificate for any additional time you will need to make an appointment to see a doctor. Further certificates can then be arranged with one of our receptionists. Where possible please contact the surgery two days before your sick note runs out (no later than the day before please).

You will be asked:
- Your full name and address
- The date you would like your sick note to start from
- Your medical condition, this information is on your current sick note
- The duration that the sick note will cover
- The GP who is dealing with your current medical condition

Please allow 48 hours before you collect your certificate. We would advise that you telephone reception before you come in to collect your sick note.
